UGD - Data Submission Form

We strongly prefer that you submit your information via e-mail as a WORD (.doc) file, WORDPERFECT (.wpd) file or as a web (.htm or.html) file . ALL files must be less than 2 Mb in size, but may contain pictures (each no greater than 150 Kb and no larger than 640 pixels wide and 480 pixels high). Please make sure your submission contains all the data elements (titles) shown in the table below.

If it is not possible to send a word processor file, you you can submit your UGD entry directly to the UGD administrators using your Web browser! Fill out the details below and click the "Submit to Database" button. Pictures can be sent separately via e-mail as attachments. We will get back to you via email or fax to confirm your entry in the Database as soon as possible. Remember - if you make an error in your data entry, just click on the "Cancel Entries" button below to reset all the fields.

At least the first four fields are required to enter a new Town or City into the Database successfully. Sample database entries for Perth, Western Australia, are provided as a guide.
