Should an emergency arise while you are at UTSC, the University Police will attempt to contact you as soon as they are able to do so. To register for this service, complete the information below, and the schedule on the reverse of this page.

Please ensure that this information is updated each term, since it provides the only means of locating you. Be sure to include the time and location of your classes, as well as other places where you can be routinely located on University, such as the study hall, the library, the cafeteria, your office, etc. Submit your schedule to the University Police office.

Please inform the appropriate people that in the event of an emergency, they should contact the University Police at 287-7333, Room S-300, inside the Science Wing Main Entrance.

REQUEST FOR ASSISTANCE DURING AN EVACUATION

Any student, staff or faculty member requiring assistance during a building evacuation has the responsibility to make this known to the University. The University cannot make an independent evaluation of such need. Assistance is usually needed by anyone with hearing, visual, some health considerations or mobility difficulties.

All information provided will be kept confidential and only the persons and agencies providing the required assistance will have access. Please fill in this form and return it as soon as possible to the University Police. Please advise the University Police when there is a change in any of this information.

Students are advised to inform their professors that they may require evacuation assistance.
